Incidents to Report:
- Unexpected resident or visitor death
- Serious injury to resident or visitor
- Medication or treatment error leading to serious injury
- Suspected physical abuse and/or neglect
- Attempted or actual suicide by resident, visitor, or staff
- Resident to resident altercation resulting in injury
- Regulatory survey with immediate jeopardy citation
When an Incident Occurs:
- Administrator or designee notifies physician and family or guardian
- Initiate an incident report with objective information only
- Document the medical record with objective information
- DO NOT refer to the incident report in the medical record
- Make no admission of liability or promises to reimburse
- Summarize witness statements as part of the investigative process
- Report the incident
