If a sentinel event has occurred at your facility, CALL US IMMEDIATELY! We have legal support standing by 24 hours a day. The best opportunity to affect the outcome of an incident is within the first hours or days after the event occurs. Remember - we are on your side! You are not penalized for reporting incidents to us! On the contrary, our underwriters give credits to those facilities that keep in touch with us.

Report the following support line immediately:

  • Unexpected resident or visitor death
  • Serious injury to resident or visitor
  • Medication or treatment error leading to serious injury
  • Suspected physical abuse and/or neglect
  • Attempted or actual suicide by resident, visitor, or staff
  • Resident to resident altercation resulting in injury
  • Regulatory survey with immediate jeopardy citation

When a serious incident occurs:

  1. Administrator or designee notifies physician and family or guardian
  2. Initiate an incident report with objective information only
  3. Document the medical record with objective information
  4. DO NOT refer to the incident report in the medical record
  5. Call the SOS Hotline at (877) 473-7773
  6. Make no admission of liability or promises to reimburse
  7. Summarize witness statements as part of the investigative process
