How to find your policy and invoice numbers

Your policy number and invoice number can be found in 2 places on your invoice:

Upper Right Corner of Invoice

Bottom of Invoice - On Remittance Stub

HealthCap Online Premium Payment

Step 1 - Enter Invoice Information
