On April 22nd, the Michigan Department of Health & Human Services (MDHHS) shared guidance and forms for submitting confirmatory Polymerase Chain Reaction (PCR) COVID tests to the Michigan Bureau of Labs (BOL). To support ongoing testing in long term care facilities, the BOL is offering the option for providers to utilize their service for confirmatory PCR tests. The BOL will run the tests and report back to providers at no charge.
Resource materials for providers is available, including:
- Electronic Test Ordering and Reporting (ETOR) User Manual
- COVID-19 PCR Test Requisition Form – MDHHS-5891
- MDHHS-5891 Form Completion and Specimen Submission Instructions
- BOL Automatic Fax Transmission Reporting Form
Due to security at the BOL, providers are advised to call ahead if choosing to deliver specimens directly to the lab. Contact the Data & Specimen Handling Unit at 517-335-8059 to advise of an approximate arrival time to avoid security processing delays.
